   A screw-up; a mistake that is somebody's fault. The verb накосячить means to make one, and косячить is to keep making them.

2. 2.

   Separately and much more narrowly, a joint of cannabis — context always disambiguates, but the two senses are unrelated in feel and rarely confused.

Where it comes from

Camp argot, where допустить косяк meant committing a breach of the code. The everyday sense has lost all of that and simply means an error you have to own.
